Uncertain significance for Cardiovascular phenotype — the classification assigned by Ambry Genetics to NM_000363.5(TNNI3):c.11+1G>A, citing Ambry Variant Classification Scheme 2023. This variant lies in the TNNI3 gene (transcript NM_000363.5) at the canonical splice donor site of the intron immediately after coding-DNA position 11, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.11+1G>A intronic variant results from a G to A substitution one nucleotide after coding exon 1 of the TNNI3 gene. This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. Although biallelic loss of function of TNNI3 has been associated with autosomal recessive dilated cardiomyopathy, haploinsufficiency of TNNI3 has not been established as a mechanism of disease for autosomal dominant cardiomyopathy. Based on the supporting evidence, this variant is expected to be causative of autosomal recessive dilated cardiomyopathy when present along with a second pathogenic variant on the other allele; however, its clinical significance for autosomal dominant cardiomyopathy is unclear.
