NM_000535.7(PMS2):c.223G>A (p.Val75Ile) was classified as Uncertain significance for Hereditary nonpolyposis colorectal neoplasms by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): This sequence change replaces valine with isoleucine at codon 75 of the PMS2 protein (p.Val75Ile). The valine residue is highly conserved and there is a small physicochemical difference between valine and isoleucine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals with a PMS2-related disease. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Tolerated"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). In summary, this variant has uncertain impact on PMS2 function. The available evidence is currently insufficient to determine its role in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
