NM_000257.4(MYH7):c.1481T>A (p.Phe494Tyr) was classified as Uncertain significance for Hypertrophic cardiomyopathy by Labcorp Genetics (formerly Invitae), Labcorp, citing Invitae Variant Classification Sherloc (09022015): In summary, this variant is a novel missense change with uncertain impact on protein function. While it is located in a region where multiple missense variants have been associated with HCM, additional genetic and/or functional evidence is needed to classify this variant conclusively. Therefore, it has been classified as a Variant of Uncertain Significance. A computational algorithm designed to assess the pathogenicity of variants in MYH7 with regard to hypertrophic cardiomyopathy predicted this sequence change to be deleterious. The algorithm has a sensitivity of 94% and a specificity of 89% (PMID: 21310275). This variant is not present in population databases (ExAC no frequency) and has not been reported in the literature in individuals with a MYH7-related disease. This variant is found within a region of MYH7 between codons 181 and 937 that contains the majority of the myosin head domain. Missense variants in this region have been shown to be significantly overrepresented in individuals with hypertrophic cardiomyopathy (PMID: 27532257). This sequence change replaces phenylalanine with tyrosine at codon 494 of the MYH7 protein (p.Phe494Tyr). The phenylalanine residue is highly conserved and there is a small physicochemical difference between phenylalanine and tyrosine.
