Pathogenic for Mucopolysaccharidosis type 1 — the classification assigned by ClinGen Lysosomal Storage Disorder Variant Curation Expert Panel to NM_000203.5(IDUA):c.792+1G>T, citing ClinGen LSD ACMG Specifications IDUA V1.1.0: The NM_000203.5:c.792+1G>T variant in IDUA occurs within the canonical splice donor site of intron 6. It is predicted to cause skipping of biologically-relevant-exon 6 out of 14, resulting in a frameshift leading to nonsense mediated decay in a gene in which loss-of-function is an established disease mechanism (PVS1). A different nucleotide change within the same splice donor dinucleotide, c.792+1G>C (PMID: 21480867), is classified as pathogenic for MPS I by the ClinGen Lysosomal Diseases VCEP (PS1_Supporting). This variant has been detected in 5 individuals with MPS I. Of those individuals, 2 were compound heterozygous for the variant and a variant in IDUA (c.1190-1G>A, ClinVarID: 1204494); the allelic data for this patient will be used to support the classification of c.1190-1G>A and is not included here in order to avoid circular logic. 2 were unable to identify the second variant. 1 individual was homozygous for the variant (0.5 points, PMID: 21734815) (PM3_Supporting). At least 5 patients with this variant had documented IDUA deficiency within the affected range in leukocytes, and clinical features specific to MPS I including hepatosplenomegaly and corneal involvement (PP4). This variant is absent in gnomAD v4.1.0. (PM2_Supporting). In summary, this variant meets the criteria to be classified as pathogenic for MPS I based on the IDUA-specific ACMG/AMP criteria applied, as specified by the ClinGen Lysosomal Diseases Variant Curation Expert Panel (Specifications Version 1.0.0): PVS1, PS1_Supporting, PM3_Supporting, PP4, PM2_Supporting.
