The duplication introduces a frameshift that results in premature truncation of the protein, leading to loss of the majority of the death domain—an essential region for EDAR function—satisfying PVS1 (null variant in a gene where loss of function is a known mechanism of disease). The variant is absent from large population databases, fulfilling PM2. In the reported family, the variant co-segregates with the tooth agenesis phenotype, consistent with PP1. The proband and affected parent exhibit a phenotype that is highly specific for EDAR-related non-syndromic tooth agenesis, meeting PP4.
