Uncertain significance — the classification assigned by GeneDx to NM_006846.4(SPINK5):c.1366G>T (p.Asp456Tyr), citing GeneDx Variant Classification (06012015): The D456Y variant in the SPINK5 gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. This variant is observed in 9/30,780 alleles (0.029%) from individuals of South Asian background in the gnomAD dataset (Lek et al., 2016). The D456Y variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. In addition, this substitution occurs at a position that is conserved across mammalian species. However, in silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. We interpret D456Y as a variant of uncertain significance.
