Likely pathogenic — the classification assigned by GeneDx to NM_022552.5(DNMT3A):c.2478+1G>T, citing GeneDx Variant Classification (06012015). This variant lies in the DNMT3A gene (transcript NM_022552.5) at the canonical splice donor site of the intron immediately after coding-DNA position 2478, where G is replaced by T; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.2478+1G>T variant in the DNMT3A gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. This splice site variant destroys the canonical splice donor site in intron 2. It is predicted to cause abnormal gene splicing, either leading to an abnormal message that is subject to nonsense-mediated mRNA decay, or to an abnormal protein product if the message is used for protein translation. The c.2478+1G>T variant is not observed in large population cohorts (Lek et al., 2016). We interpret c.2478+1G>T as a likely pathogenic variant.
