NM_004380.3(CREBBP):c.3743A>T (p.Asn1248Ile) was classified as Likely pathogenic by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the CREBBP gene (transcript NM_004380.3) at coding-DNA position 3743, where A is replaced by T; at the protein level this means replaces asparagine at residue 1248 with isoleucine — a missense variant. Submitter rationale: The N1248I variant in the CREBBP gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The N1248I variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The N1248I variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position where amino acids with similar properties to Asparagine are tolerated across species. In silico analysis predicts this variant is probably damaging to the protein structure/function. We interpret N1248I as a likely pathogenic variant.
