This variant lies in the KMT2D gene (transcript NM_003482.4) at coding-DNA position 571, where C is replaced by T; at the protein level this means replaces arginine at residue 191 with tryptophan — a missense variant. Submitter rationale: The R191W variant in the KMT2D gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. This variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). Although in silico analysis is inconsistent in its predictions as to whether or not the R191W variant is damaging to the protein structure/function, this variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. Additionally, this substitution occurs at a position that is conserved across species. We interpret R191W as a likely pathogenic variant.
