Pathogenic — the classification assigned by GeneDx to NM_000193.4(SHH):c.1258dup (p.Ala420fs), citing GeneDx Variant Classification (06012015): The c.1258dupG pathogenic variant in the SHH gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. The c.1258dupG variant causes a frameshift starting with codon Alanine 420, changes this amino acid to a Glycine residue and replaces the typical last 43 amino acid residues in the sonic hedgehog protein with an unknown number of different amino acid residues, which can be described as p.Ala420GlyfsX?. This change is expected to alter the normal structure and function of the resultant protein. The c.1258dupG variant was not observed in the Exome Aggregation Consortium (ExAC) data set, indicating it is not a common benign variant. We interpret c.1258dupG as a pathogenic variant.
