Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_007294.4(BRCA1):c.4449T>A (p.Ser1483Arg), citing Ambry Variant Classification Scheme 2023. This variant lies in the BRCA1 gene (transcript NM_007294.4) at coding-DNA position 4449, where T is replaced by A; at the protein level this means replaces serine at residue 1483 with arginine — a missense variant. Submitter rationale: The p.S1483R variant (also known as c.4449T>A), located in coding exon 12 of the BRCA1 gene, results from a T to A substitution at nucleotide position 4449. The serine at codon 1483 is replaced by arginine, an amino acid with dissimilar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6502 samples (13004 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.0004% (greater than 225000 alleles tested) in our clinical cohort. This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
