Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_007294.4(BRCA1):c.3132T>G (p.Ile1044Met), citing Ambry Variant Classification Scheme 2023. This variant lies in the BRCA1 gene (transcript NM_007294.4) at coding-DNA position 3132, where T is replaced by G; at the protein level this means replaces isoleucine at residue 1044 with methionine — a missense variant. Submitter rationale: The p.I1044M variant (also known as c.3132T>G), located in coding exon 9 of the BRCA1 gene, results from a T to G substitution at nucleotide position 3132. The isoleucine at codon 1044 is replaced by methionine, an amino acid with highly similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6502 samples (13004 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.0003% (greater than 300000 alleles tested) in our clinical cohort. This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be possibly damaging and deleterious by PolyPhen and SIFT in silico analyses, respectively. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
