Pathogenic for Familial cancer of breast — the classification assigned by Labcorp Genetics (formerly Invitae), Labcorp to NC_000016.10:g.(?_23603459)_(23626397_?)del, citing Invitae Variant Classification Sherloc (09022015): This variant is a gross deletion of the genomic region encompassing exons 7-13 of the PALB2 gene. The 5' boundary is likely confined to intron 6. The 3' end of this event is unknown as it extends through the termination codon beyond the assayed region for this gene and may encompass additional genes. While this deletion is not anticipated to result in nonsense mediated decay, it is expected to create a truncated protein product or disrupt mRNA translation. Exons 7-13 of PALB2 encodes the WD40 domain which associates with the N-terminus of BRCA2 (PMID: 19423707). This deletion destroys the WD40 functional domain. While this exact sequence change has not been reported in the literature, sub-genic deletion spanning exons 12-13 of PALB2 has been reported in an individual affected with breast and pancreatic cancer (PMID: 19635604) and a sub-genic deletion spanning exons 7-11 of PALB2 has been reported in an individual affected with breast cancer and their brother affected with breast and gastric cancer (PMID: 22052327). In addition, loss of function mutations causing partial deletion of the WD40 domain have been classified as pathogenic (Invitae internal database). For these reasons, this sequence change has been classified as Pathogenic.
