This variant lies in the RYR1 gene (transcript NM_000540.3) at coding-DNA position 12575, where T is replaced by C; at the protein level this means replaces isoleucine at residue 4192 with threonine — a missense variant. Submitter rationale: The I4192T variant in the RYR1 gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The I4192T variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The I4192T variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position where amino acids with similar properties to Isoleucine are tolerated across species. In silico analysis predicts this variant is probably damaging to the protein structure/function. We interpret I4192T as a variant of uncertain significance.
