Uncertain significance — the classification assigned by GeneDx to NM_001372044.2(SHANK3):c.1175A>G (p.Gln392Arg), citing GeneDx Variant Classification (06012015). This variant lies in the SHANK3 gene (transcript NM_001372044.2) at coding-DNA position 1175, where A is replaced by G; at the protein level this means replaces glutamine at residue 392 with arginine — a missense variant. Submitter rationale: The Q317R variant in the SHANK3 gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The Q317R variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The Q317R variant is a semi-conservative amino acid substitution, which may impact secondary protein structure as these residues differ in some properties. This substitution occurs at a position that is conserved across species. In silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. We interpret Q317R as a variant of uncertain significance.
