Uncertain significance — the classification assigned by GeneDx to NM_001079.4(ZAP70):c.192G>C (p.Gln64His), citing GeneDx Variant Classification (06012015). This variant lies in the ZAP70 gene (transcript NM_001079.4) at coding-DNA position 192, where G is replaced by C; at the protein level this means replaces glutamine at residue 64 with histidine — a missense variant. Submitter rationale: The Q64H variant in the ZAP70 gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The Q64H variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The Q64H variant is a semi-conservative amino acid substitution, which may impact secondary protein structure as these residues differ in some properties. Although this substitution occurs at a position that is conserved across species, in silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. We interpret Q64H as a variant of uncertain significance.
