Uncertain significance — the classification assigned by GeneDx to NM_006059.4(LAMC3):c.4348C>T (p.Arg1450Cys), citing GeneDx Variant Classification (06012015). This variant lies in the LAMC3 gene (transcript NM_006059.4) at coding-DNA position 4348, where C is replaced by T; at the protein level this means replaces arginine at residue 1450 with cysteine — a missense variant. Submitter rationale: A variant of uncertain significance has been identified in the LAMC3 gene. The R1450C variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. The R1450C variant is observed in 74/16,508 (0.4%) alleles from individuals of South Asian background including multiple unrelated homozygous individuals in large population cohorts, which is greater than expected for this disorder (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The R1450C variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position where amino acids with similar properties to Arginine are tolerated across species. In silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic variant or a rare benign variant.
