Likely pathogenic — the classification assigned by GeneDx to NM_001846.4(COL4A2):c.4039+1G>A, citing GeneDx Variant Classification (06012015). This variant lies in the COL4A2 gene (transcript NM_001846.4) at the canonical splice donor site of the intron immediately after coding-DNA position 4039, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.4039+1G>A variant in the COL4A2 gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. This splice site variant destroys the canonical splice donor site in intron 42. It is predicted to cause abnormal gene splicing, either leading to an abnormal message that is subject to nonsense-mediated mRNA decay, or to an abnormal protein product if the message is used for protein translation. The c.4039+1G>A variant was not observed in approximately 6000 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. However, the vast majority of pathogenic variants reported in COL4A2 in the Human Gene Mutation database (HGMD) are missense variants (Stenson et al., 2014), therefore the pathogenicity of this and other truncating variants has not been completely established.
