NM_000090.4(COL3A1):c.2381G>A (p.Arg794His) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the COL3A1 gene (transcript NM_000090.4) at coding-DNA position 2381, where G is replaced by A; at the protein level this means replaces arginine at residue 794 with histidine — a missense variant. Submitter rationale: A variant of uncertain significance has been identified in the COL3A1 gene. The R794H variant has not been published as pathogenic or been reported as benign to our knowledge. This variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The R794H substitution occurs at a position that is conserved across species and in silico analysis predicts this variant is probably damaging to the protein structure/function. Nevertheless, the R794H variant is a conservative amino acid substitution, which is not likely to impact secondary protein structure as these residues share similar properties. Additionally, although the R794H variant is located in the triple helical region of the COL3A1 gene, it does not affect a Glycine residue in a Gly-X-Y motif, where the majority of pathogenic missense variants occur (Stenson et al., 2014; Symoens et al., 2012).
