NM_000383.4(AIRE):c.34A>G (p.Arg12Gly) was classified as Likely pathogenic by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the AIRE gene (transcript NM_000383.4) at coding-DNA position 34, where A is replaced by G; at the protein level this means replaces arginine at residue 12 with glycine — a missense variant. Submitter rationale: To our knowledge, the R12G variant has not been published as a pathogenic variant, nor has it been reported as a benign variant. The R12G variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). It is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position within the HSR domain where amino acids with similar properties to Arginine are tolerated across species. In silico analysis predicts this variant is probably damaging to the protein structure/function. Additionally, missense variants in nearby residues (R8C, R15C/L, T16M) have been reported in the Human Gene Mutation Database in association with APECED (Stenson et al., 2014), supporting the functional importance of this region of the protein. In summary, we consider this variant to be likely pathogenic.
