Likely pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000051.4(ATM):c.2638+1del, citing Ambry Variant Classification Scheme 2023. This variant lies in the ATM gene (transcript NM_000051.4) at the canonical splice donor site of the intron immediately after coding-DNA position 2638, deleting one base. Submitter rationale: The c.2638+1delG intronic variant results from the deletion of one nucleotide (G) at position c.2638+1, which is located one nucleotide after coding exon 16 of the ATM gene. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6499 samples (12998 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.002% (greater than 66,000 alleles tested) in our clinical cohort. The BDGP splice site prediction tool predicts this alteration will shift the splice donor site one nucleotide upstream from the native site; however, direct experimental evidence is unavailable. Alterations that disrupt the canonical splice donor site are typically deleterious in nature (ACMG Recommendations for Standards for Interpretation and Reporting of Sequence Variations. Revision 2007. Genet Med. 2008;10:294). As such, the c.2638+1delG variant is classified as likely pathogenic.
