NM_000257.4(MYH7):c.2093T>C (p.Val698Ala) was classified as Likely pathogenic for Hypertrophic cardiomyopathy 1 by Agnes Ginges Centre for Molecular Cardiology, Centenary Institute, citing ACMG Guidelines, 2015: MYH7 Val698Ala was first described in our HCM proband (Ingles et al., 2005) and has now been identified in 1 additional proband with childhood onset HCM (Maurizi et al., 2018). Genetic analysis of our family found the variant to segregate with disease (6 meioses). This variant is absent in the Genome Aggregation Database (http://gnomad.broadinstitute.org/). In a large HCM population study Walsh et al., showed that MYH7 variants identified in HCM cases were found to cluster between amino acids 181- 937 (2017), this implies that variants in this region are likely to cause a HCM phenotype. Furthermore variants located in the converter region of MYH7 are predicted to result in worse outcomes (Garcia-Giustiniani et al., 2015). Computational tools SIFT, PolyPhen-2 and MutationTaster predict this variant to be deleterious. Based on the adapted ACMG criteria (Kelly MA, et al., 2018) this variant is located in a mutational hotspot (PM1), is rare in the general population (PM2), segregates with disease (PP1_Mod/Strong?) and multiple in silico tools predict it to be deleterious (PP3), therefore we classify MYH7 Val698Ala as "pathogenic".
