NM_000179.3(MSH6):c.949A>G (p.Lys317Glu) was classified as Uncertain significance for Hereditary cancer-predisposing syndrome by Ambry Genetics, citing Ambry Variant Classification Scheme 2023. This variant lies in the MSH6 gene (transcript NM_000179.3) at coding-DNA position 949, where A is replaced by G; at the protein level this means replaces lysine at residue 317 with glutamic acid — a missense variant. Submitter rationale: The p.K317E variant (also known as c.949A>G), located in coding exon 4 of the MSH6 gene, results from an A to G substitution at nucleotide position 949. The lysine at codon 317 is replaced by glutamic acid, an amino acid with similar properties. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 150000 alleles tested) in our clinical cohort. This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. In addition, the CoDP in silico tool predicts this alteration to have minor impact on molecular function, with a score of 0.000 (Terui H et al. J. Biomed. Sci. 2013;20:25). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
