NM_004444.5(EPHB4):c.1470C>G (p.Asn490Lys) was classified as Uncertain significance for Capillary malformation-arteriovenous malformation 2 by Clinical Genomics Laboratory, Washington University in St. Louis, citing ACMG Guidelines, 2015. This variant lies in the EPHB4 gene (transcript NM_004444.5) at coding-DNA position 1470, where C is replaced by G; at the protein level this means replaces asparagine at residue 490 with lysine — a missense variant. Submitter rationale: An EPHB4 c.1470C>G (p.Asn490Lys) variant was identified at a near heterozygous allelic fraction of 50.5%, a frequency which may be consistent with it being of germline origin. This variant, to our knowledge, has not been reported in the medical literature. It is only observed on 2/1,590,472 alleles in the general population (gnomAD v.4.1.0), indicating it is not a common variant. Computational predictors suggest that the variant does not impact EPHB4 function. Due to limited information, and based on ACMG/AMP guidelines for variant interpretation (Richards S et al., PMID: 25741868), the clinical significance of the EPHB4 c.1470C>G (p.Asn490Lys) variant is uncertain at this time.
