This variant lies in the SCN8A gene (transcript NM_001330260.2) at coding-DNA position 4892, where T is replaced by A; at the protein level this means replaces isoleucine at residue 1631 with asparagine — a missense variant. Submitter rationale: The I1631N variant has not been published as a pathogenic variant, nor has it been reported as a benign polymorphism to our knowledge. It was not observed in approximately 6,400 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The I1631N variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution alters a conserved position predicted to be in the transmembrane segment S4 (voltage sensor) in the fourth homologous domain, and in silico analysis predicts this variant is probably damaging to the protein structure/function. Additionally, a de novo missense variant in an adjacent residue (L1630P) was identified previously in another individual tested at GeneDx.
