NM_033419.5(PGAP3):c.380C>T (p.Thr127Ile) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the PGAP3 gene (transcript NM_033419.5) at coding-DNA position 380, where C is replaced by T; at the protein level this means replaces threonine at residue 127 with isoleucine — a missense variant. Submitter rationale: The T127I variant in the PGAP3 gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The T127I variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The T127I variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. However, this substitution occurs at a position that is not conserved. In silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. We interpret T127I as a variant of uncertain significance.
