NM_000391.4(TPP1):c.1045G>A (p.Ala349Thr) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015): A variant of uncertain significance has been identified in the TPP1 gene. The A349T variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. The A349T variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The A349T variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. Multiple missense variants in nearby residues have been reported in Human Gene Mutation Database in association with late-infantile neuronal ceroid lipofuscinosis (Stenson et al., 2014), supporting the functional importance of this region of the protein. However, this substitution occurs at a position that is not conserved and in silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. Therefore, based on the currently available information, it is unclear whether this variant is a pathogenic variant or a rare benign variant.
