NM_004006.3(DMD):c.652G>A (p.Val218Ile) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the DMD gene (transcript NM_004006.3) at coding-DNA position 652, where G is replaced by A; at the protein level this means replaces valine at residue 218 with isoleucine — a missense variant. Submitter rationale: A variant of uncertain significance has been identified in the DMD gene. The V218I variant has not been published as pathogenic or been reported as benign to our knowledge. This variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). However, the V218I variant is a conservative amino acid substitution, which is not likely to impact secondary protein structure as these residues share similar properties. This substitution occurs at a position where amino acids with similar properties to valine are tolerated across species and where isoleucine is present as the wild type in at least one species. In silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. Finally, while a missense variant in the same residue (V218G) has been reported in association with BMD, the pathogenicity of this variant has not been definitively determined (Nicolas et al., 2012).
