Pathogenic — the classification assigned by GeneDx to NM_001032221.6(STXBP1):c.420T>A (p.Tyr140Ter), citing GeneDx Variant Classification (06012015). This variant lies in the STXBP1 gene (transcript NM_001032221.6) at coding-DNA position 420, where T is replaced by A; at the protein level this means converts the codon for tyrosine at residue 140 into a premature stop signal — a nonsense variant expected to truncate the protein. Submitter rationale: The Y140X variant in the STXBP1 gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The Y140X variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). Therefore, we interpret Y140X as a pathogenic variant
