NM_139276.3(STAT3):c.1943T>A (p.Met648Lys) was classified as Likely pathogenic by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the STAT3 gene (transcript NM_139276.3) at coding-DNA position 1943, where T is replaced by A; at the protein level this means replaces methionine at residue 648 with lysine — a missense variant. Submitter rationale: The M648K variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. The variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). M648K is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position within the critical SH2 domain that is conserved across species; the SH2 domain mediates dimerization and activation of the STAT3 protein and is a hotspot for variants (Chandesris et al., 2012). In silico analysis predicts this variant is probably damaging to the protein structure/function. In summary, this variant is likely pathogenic; however, the possibility that it is benign cannot be excluded.
