Uncertain significance — the classification assigned by GeneDx to NM_000719.7(CACNA1C):c.1841T>G (p.Leu614Arg), citing GeneDx Variant Classification (06012015). This variant lies in the CACNA1C gene (transcript NM_000719.7) at coding-DNA position 1841, where T is replaced by G; at the protein level this means replaces leucine at residue 614 with arginine — a missense variant. Submitter rationale: The L614R variant in the CACNA1C gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The L614R variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The L614R variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. In addition, this substitution occurs at a position that is conserved across species, and in silico analysis predicts this variant is probably damaging to the protein structure/function. We interpret L614R as a variant of uncertain significance.
