Pathogenic — the classification assigned by GeneDx to NM_001347721.2(DYRK1A):c.1445_1446delinsTCTACA (p.Asn482fs), citing GeneDx Variant Classification (06012015). This variant lies in the DYRK1A gene (transcript NM_001347721.2) at coding-DNA position 1445 through coding-DNA position 1446, replacing the reference sequence with TCTACA; at the protein level this means shifts the reading frame starting at asparagine residue 482, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The c.1472_1473delATinsTCTACA variant in the DYRK1A gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. This variant causes a frameshift starting with codon Asparagine 491, changes this amino acid to a Isoleucine residue, and creates a premature Stop codon at position 83 of the new reading frame, denoted p.Asn491IlefsX83. This variant is predicted to cause loss of normal protein function through protein truncation, as the last 273 amino acids of the protein are replaced by 82 incorrect amino acids. The c.1472_1473delATinsTCTACA variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). We interpret c.1472_1473delATinsTCTACA as a pathogenic variant.
