Uncertain significance — the classification assigned by GeneDx to NM_000530.8(MPZ):c.610A>G (p.Lys204Glu), citing GeneDx Variant Classification (06012015). This variant lies in the MPZ gene (transcript NM_000530.8) at coding-DNA position 610, where A is replaced by G; at the protein level this means replaces lysine at residue 204 with glutamic acid — a missense variant. Submitter rationale: The K204E variant in the MPZ gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The K204E variant is not observed in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The K204E variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved in mammals. However, in silico analysis is inconsistent in its predictions as to whether or not the variant is damaging to the protein structure/function. We interpret K204E as a variant of uncertain significance.
