This variant lies in the POLE gene (transcript NM_006231.4) at coding-DNA position 1156, where G is replaced by A; at the protein level this means replaces glutamic acid at residue 386 with lysine — a missense variant. Submitter rationale: This variant is denoted POLE c.1156G>A at the cDNA level, p.Glu386Lys (E386K) at the protein level, and results in the change of a Glutamic Acid to a Lysine (GAG>AAG). This variant has not, to our knowledge, been published in the literature as either a pathogenic germline variant or a benign polymorphism. However, it has been reported as a somatic variant in malignant melanoma (COSMIC database). POLE Glu386Lys was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, suggesting it is not a common benign variant in these populations. Since Glutamic Acid and Lysine differ in polarity, charge, size or other properties, this is considered a non-conservative amino acid substitution. POLE Glu386Lys occurs at a position that is conserved across species and is not located in a known functional domain (Tahirov 2009, Preston 2010). In silico analyses predict that this variant is probably damaging to protein structure and function. Based on currently available evidence, it is unclear whether POLE Glu386Lys is a pathogenic or benign variant. We consider it to be a variant of uncertain significance.
