Likely pathogenic — the classification assigned by GeneDx to NM_004281.4(BAG3):c.350dup (p.Gly118fs), citing GeneDx Variant Classification (06012015). This variant lies in the BAG3 gene (transcript NM_004281.4) at coding-DNA position 350, duplicating one base; at the protein level this means shifts the reading frame starting at glycine residue 118, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: The c.350dupC variant in the BAG3 gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. The c.350dupC variant causes a frameshift starting with codon Glycine 118, changes this amino acid to a Tryptophan residue, and creates a premature Stop codon at position 8 of the new reading frame, denoted p.Gly118TrpfsX8. This variant is predicted to cause loss of normal protein function either through protein truncation or nonsense-mediated mRNA decay. The c.350dupC variant was not observed in approximately 6500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The c.350dupC variant is a strong candidate for a pathogenic variant
