NM_198576.4(AGRN):c.5743C>T (p.Arg1915Trp) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the AGRN gene (transcript NM_198576.4) at coding-DNA position 5743, where C is replaced by T; at the protein level this means replaces arginine at residue 1915 with tryptophan — a missense variant. Submitter rationale: The R1915W variant in the AGRN gene has not been reported previously as a pathogenic variant, nor as a benign variant, to our knowledge. The R1915W variant is not observed at a significant frequenc in large population cohorts (Lek et al., 2016; 1000 Genomes Consortium et al., 2015; Exome Variant Server). The R1915W variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved across species, and in silico analysis predicts this variant is probably damaging to the protein structure/function. We interpret R1915W as a variant of uncertain significance.
