NM_000551.4(VHL):c.169G>C (p.Gly57Arg) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the VHL gene (transcript NM_000551.4) at coding-DNA position 169, where G is replaced by C; at the protein level this means replaces glycine at residue 57 with arginine — a missense variant. Submitter rationale: This variant is denoted VHL c.169G>C at the cDNA level, p.Gly57Arg (G57R) at the protein level, and results in the change of a Glycine to an Arginine (GGG>CGG). This variant has not, to our knowledge, been published in the literature as pathogenic or benign. VHL Gly57Arg was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, suggesting it is not a common benign variant in these populations. Since Glycine and Arginine differ in polarity, charge, size or other properties, this is considered a non-conservative amino acid substitution. VHL Gly57Arg occurs at a position that is not conserved and is not located in a known functional domain (Yuen 2009). In silico analyses are inconsistent regarding the effect this variant may have on protein structure and function. Based on currently available evidence, it is unclear whether VHL Gly57Arg is a pathogenic or benign variant. We consider it to be a variant of uncertain significance.
