NM_024675.4(PALB2):c.2411C>T (p.Ser804Phe) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015). This variant lies in the PALB2 gene (transcript NM_024675.4) at coding-DNA position 2411, where C is replaced by T; at the protein level this means replaces serine at residue 804 with phenylalanine — a missense variant. Submitter rationale: This variant is denoted PALB2 c.2411C>T at the cDNA level, p.Ser804Phe (S804F) at the protein level, and results in the change of a Serine to a Phenylalanine (TCT>TTT). This variant has been observed in at least one individual with ovarian cancer (Ramus 2015). PALB2 Ser804Phe was not observed at a significant allele frequency in the NHLBI Exome Sequencing Project. Since Serine and Phenylalanine differ in polarity, charge, size or other properties, this is considered a non-conservative amino acid substitution. PALB2 Ser804Phe occurs at a position that is not conserved and is located in a region required for interaction with POLH and POLH DNA synthesis stimulation (UniProt). In silico analyses are inconsistent regarding the effect this variant may have on protein structure and function. Based on currently available evidence, it is unclear whether PALB2 Ser804Phe is a pathogenic or benign variant. We consider it to be a variant of uncertain significance.
