Uncertain significance — the classification assigned by GeneDx to NM_007294.4(BRCA1):c.1580A>G (p.Lys527Arg), citing GeneDx Variant Classification (06012015): This variant is denoted BRCA1 c.1580A>G at the cDNA level, p.Lys527Arg (K527R) at the protein level, and results in the change of a Lysine to an Arginine (AAG>AGG). Using alternate nomenclature, this variant would be defined as BRCA1 1699A>G. This variant has not, to our knowledge, been published in the literature as pathogenic or benign. BRCA1 Lys527Arg was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. Since Lysine and Arginine share similar properties, this is considered a conservative amino acid substitution. BRCA1 Lys527Arg occurs at a position that is not conserved and is located in the DNA binding domain (Narod 2004). In silico analyses predict that this variant is unlikely to alter protein structure or function. Based on currently available information, it is unclear whether BRCA1 Lys527Arg is pathogenic or benign. We consider it to be a variant of uncertain significance.
