Likely pathogenic — the classification assigned by GeneDx to NM_144736.5(NDUFAF7):c.644A>G (p.His215Arg), citing GeneDx Variant Classification (06012015): A novel H215R missense change that is likely pathogenic was identified in the NDUFAF7 gene. It has not been published as a pathogenic variant, nor has it been reported as a benign polymorphism to our knowledge. H215R is a conservative amino acid substitution as Histidine and Arginine are both positively charged residues; however, the variant alters a highly conserved position in the protein and multiple in silico algorithms predict that H215R is damaging to the structure/function of the protein. Therefore, H215R is a strong candidate for a pathogenic variant, however the possibility that it is a benign variant cannot be excluded.
