NM_000179.3(MSH6):c.1716G>T (p.Gln572His) was classified as Uncertain significance by GeneDx, citing GeneDx Variant Classification (06012015): This variant is denoted MSH6 c.1716G>T at the cDNA level, p.Gln572His (Q572H) at the protein level, and results in the change of a Glutamine to a Histidine (CAG>CAT). This variant has not, to our knowledge, been published in the literature as pathogenic or benign. MSH6 Gln572His was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, suggesting it is not a common benign variant in these populations. Since Glutamine and Histidine differ in some properties, this is considered a semi-conservative amino acid substitution. MSH6 Gln572His occurs at a position that is conserved across species and is located in the MSH2 binding site and domain II of the MutS domain (Kariola 2002, Terui 2013). In silico analyses predict that this variant is probably damaging to protein structure and function. Based on currently available information, it is unclear whether MSH6 Gln572His is pathogenic or benign. We consider it to be a variant of uncertain significance.
