NM_000975.5(RPL11):c.508-1G>C was classified as Likely pathogenic for Diamond-Blackfan anemia by Ambry Genetics, citing Ambry Variant Classification Scheme 2023: The c.508-1G>C intronic results from a G to C substitution one nucleotide before coding exon 6 of the RPL11 gene. This alteration occurs at the 3' terminus of the RPL11 gene, is not expected to trigger nonsense-mediated mRNA decay, and only impacts the last 5% of the protein. The exact functional effect of this alteration is unknown; however, a significant portion of the protein is affected (Ambry internal data). This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species. RNA studies suggest that this alteration may result in full intron retention; however, technical limitations of the assay prevent confirmation of this result. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site. Based on the available evidence, this alteration is classified as likely pathogenic.
