Pathogenic for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_000264.5(PTCH1):c.1504-2A>T, citing Ambry Variant Classification Scheme 2023. This variant lies in the PTCH1 gene (transcript NM_000264.5) at the canonical splice acceptor site of the intron immediately before coding-DNA position 1504, where A is replaced by T; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.1504-2A>T intronic pathogenic mutation results from an A to T substitution two nucleotides upstream from coding exon 11 in the PTCH1 gene. Alterations that disrupt the canonical splice site are expected to result in aberrant splicing. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site. A resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNAdecay; although, direct evidence is unavailable. However, the region predicted to be impacted is critical for protein function (Ambry internal data).This variant was reported in an at least one individual with features consistent with nevoid basal cell carcinoma syndrome (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species. Based on the supporting evidence, this variant is interpreted as a disease-causing mutation.
