Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_003000.3(SDHB):c.736A>G (p.Ile246Val), citing Ambry Variant Classification Scheme 2023. This variant lies in the SDHB gene (transcript NM_003000.3) at coding-DNA position 736, where A is replaced by G; at the protein level this means replaces isoleucine at residue 246 with valine — a missense variant. Submitter rationale: The p.I246V variant (also known as c.736A>G), located in coding exon 7 of the SDHB gene, results from an A to G substitution at nucleotide position 736. The isoleucine at codon 246 is replaced by valine, an amino acid with highly similar properties. In a study of whole-exome sequencing in patients with features of Cowden syndrome (CS) or Bannayan-Riley-Ruvalcaba syndrome (BRRS) and negative PTEN testing, this alteration was identified in 0/87 patients with CS or BRRS and 1/3476 patients from The Cancer Genome Atlas (TCGA) (Yehia L et al. PLoS Genet, 2018 04;14:e1007352). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
