Uncertain significance for Hereditary cancer-predisposing syndrome — the classification assigned by Ambry Genetics to NM_004656.4(BAP1):c.255+1G>T, citing Ambry Variant Classification Scheme 2023. This variant lies in the BAP1 gene (transcript NM_004656.4) at the canonical splice donor site of the intron immediately after coding-DNA position 255, where G is replaced by T; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The c.255+1G>T intronic variant results from a G to T substitution one nucleotide after coding exon 4 of the BAP1 gene. This alteration was non-functional in a high throughput genome editing haploid cell survival functional assay (Waters AJ et al. Nat Genet, 2024 Jul;56:1434-1445). Alterations that disrupt the canonical splice site are expected to result in aberrant splicing. In silico splice site analysis predicts that this alteration will weaken the native splice donor site and may result in the creation or strengthening of a novel splice donor site. A resulting transcript is predicted to be in-frame and is not expected to trigger nonsense-mediated mRNAdecay; although, direct evidence is unavailable.
