Pathogenic for RPGR-related retinopathy — the classification assigned by ClinGen X-linked Inherited Retinal Disease Variant Curation Expert Panel, ClinGen to NM_001034853.2(RPGR):c.2330_2331del (p.Lys777fs), citing ClinGen X LinkedIRD ACMG Specifications RPGR V1.0.0. This variant lies in the RPGR gene (transcript NM_001034853.2) at coding-DNA position 2330 through coding-DNA position 2331, deleting 2 bases; at the protein level this means shifts the reading frame starting at lysine residue 777, producing a truncated or aberrant protein — a frameshift variant. Submitter rationale: NM_001034853.2(RPGR):c.2330_2331del (p.Lys777ArgfsTer?) is a frameshift variant due to deletion of 2 nucleotides that introduces a premature stop codon within exon 15 of 15 that is predicted to disrupt a critical C-terminal region required for proper glutamylation of RPGR (PVS1, PMID: 36445968). This variant is absent from gnomAD v4.1.0 (PM2_Supporting). At least one proband harboring this variant exhibits a phenotype including myopia (0.5 pts), first/second-decade onset of disease (1 pt), family inheritance consistent with X-linked inheritance (2 pts), and night blindness (0.5 pts), which together are specific for RPGR-related retinopathy (4 points, PMID: 20806050, PP4). The variant has been reported to segregate with retinal dystrophy through at least 3 affected meioses from 1 family (PP1_Moderate; PMID: 20806050). In summary, this variant is classified as pathogenic for RPGR-related retinopathy based on the ClinGen X-linked Inherited Retinal Disease Expert Panel Specifications to the ACMG/AMP Variant Interpretation Guidelines for RPGR Version 1.0.0; PVS1, PM2_Supporting, PP1_Moderate, and PP4.
