Pathogenic for Insulin-resistant diabetes mellitus AND acanthosis nigricans — the classification assigned by Department of Traditional Chinese Medicine, Fujian Provincial Hospital to NM_000208.4(INSR):c.3734T>A (p.Val1245Glu), citing ACMG Guidelines, 2015. This variant lies in the INSR gene (transcript NM_000208.4) at coding-DNA position 3734, where T is replaced by A; at the protein level this means replaces valine at residue 1245 with glutamic acid — a missense variant. Submitter rationale: Initially, this variant was classified as "Likely Pathogenic" based on the American College of Medical Genetics and Genomics (ACMG) guidelines with evidence codes PS2 (De novo in a patient with the disease and no family history), PM2 Supporting (Absent from controls in Exome Sequencing Project, 1000 Genomes or ExAC), and PP3 (Multiple lines of computational evidence support a deleterious effect on the gene or gene product). However, in our recent study, we have gathered additional functional data that further supports upgrading its classification to "Pathogenic." Our findings include the following: SWISS-MODEL predictions indicate that the p.V1245E mutation alters the three-dimensional conformation of the INSR protein, potentially impairing its normal function. RT-qPCR analysis revealed a significant decrease in INSR mRNA expression levels in the proband. In a lentivirus-transfected 293T cell model expressing p.V1245E, Western Blot and RT-qPCR results demonstrated reduced expression of both INSR mRNA and protein. Immunofluorescence assays further confirmed decreased INSR protein expression and altered cellular localization. Based on these results, we believe the evidence now meets the criteria for upgrading the variant’s pathogenicity classification to "Pathogenic" according to ACMG guidelines.
