Uncertain significance for Neonatal ichthyosis-sclerosing cholangitis syndrome — the classification assigned by Kasturba Medical College, Manipal, Kasturba Medical College, Manipal, Manipal Academy of Higher Education, Manipal, India to NM_021101.5(CLDN1):c.272G>T (p.Gly91Val), citing ACMG Guidelines, 2015: A novel missense variant, c.272G>T in exon 2 of CLDN1 is identified in homozygous state in proband. Sanger validation and segregation analysis showed that the variant was present in homozygous state in the proband, and in heterozygous state in her parents. This variant is not reported in population databases like gnomAD (v4.1.0) or in our in-house data of 3717 exomes. In silico analysis tools (CADD_phred, REVEL) are consistent in predicting this variant as damaging to the CLDN1 protein function. The clinical findings observed in proband overlap with ichthyosis, leukocyte vacuoles, alopecia, and sclerosing cholangitis.
