NM_000284.4(PDHA1):c.831+1G>A was classified as Pathogenic for Pyruvate dehydrogenase E1-alpha deficiency by PDHA1 Study Group, University Children’s Hospital, Paracelsus Medical University. This variant lies in the PDHA1 gene (transcript NM_000284.4) at the canonical splice donor site of the intron immediately after coding-DNA position 831, where G is replaced by A; at the protein level this means a change at this position may disrupt normal splicing. Submitter rationale: The NM_000284.3:c.831+1G>A (p.?) variant affects splicing in PDHA1 gene. In total, 3 individuals were diagnosed with PDHA1-related Pyruvate dehydrogenase complex (PDHc) deficiency (MIM #312170). These include 3 females. The variant has been reported in 2 published cases (PMIDs: 28918066). Additional 1 unpublished case from internal data is included. Last literature search: July 12, 2024. This variant is absent or extremely rare in population-based cohorts in the Genome Aggregation Database (gnomAD). Individuals harboring this variant presented with clinical features compatible with PDHA1-related PDHc deficiency. In summary, this variant meets criteria to be classified as pathogenic (P) for PDHA1-related PDHc deficiency based on the ACMG/AMP criteria applied: PVS1, PS3, PM2, PM7 (last assessment October 15, 2024).
