This variant lies in the FBN1 gene (transcript NM_000138.5) at coding-DNA position 6695, where G is replaced by A; at the protein level this means replaces cysteine at residue 2232 with tyrosine — a missense variant. Submitter rationale: The p.Cys2232Tyr variant in FBN1 has been identified in at least 2 individuals with Marfan syndrome (Pepe 1997, Attanasio 2008, Proost 2005) and one individual with thoracic aortic dissection or rupture (Wolford 2018 - preprint), and was absent from large population studies. In addition, 2 other variants at the same position (p.Cys2232Arg and p.Cys2232Ser) have also been identified in individuals with Marfan syndrome, suggesting that changes at this position are not tolerated. Furthermore, this variant affects a highly conserved cysteine residue in the EGF-like domains, which is a common finding in individuals with Marfan syndrome (Schrijver 1999). In summary, although additional studies are required to fully establish its clinical significance, this variant meets criteria to be classified as likely pathogenic for autosomal dominant Marfan syndrome.
